📖 HOPC Example

Patient: 25-year-old female

Condition: Right lateral ankle pain (5 days)

Cause: Landed awkwardly in basketball game

Symptoms: Immediate pain post-injury


🔥 Pain Assessment

🔹 Type – Sharp, dull, throbbing?

🔹 Intensity – 0-10 scale

🔹 Duration – Constant or intermittent?

🔹 Pattern – Worsens/improves with activities?

📌 Example:

  • At rest – dull ache 4/10
  • Weight bearing – sharp 7/10
  • Improves with NSAIDs


⚖️ Aggravating & Easing Factors

Aggravating: Weight-bearing, walking, stairs, STS

Easing: Rest, ice, elevation, NSAIDs


⏳ 24-Hour Symptoms

🌞 Morning: Stiff with mild swelling, eases with movement

🌜 Evening: Worsens with prolonged weight-bearing


🌙 Night Pain

📌 Questions to Ask:

🔹 Do you wake up due to pain?

🔹 Is there a painful sleeping position?

🔹 Does rolling in bed cause pain?


❓ Special Questions

🟢 General Health: Any medications? Chronic conditions?

🟠 Weight Loss: Unexplained?

🔴 Imaging: X-ray/MRI?

Neurological Signs: Pins & needles? Numbness? Bowel/bladder issues?


🏃‍♂️ Objective Assessment

🔍 Observation:

  • Weight-shifting, posture, swelling
  • Functional tasks: Gait, SLS, STS, hopping

🔄 Range of Motion (AROM/PROM):

  • Limited due to pain

💪 Muscle Strength Test:

  • 🦵 Ankle Dorsiflexion – 2/5
  • 🦶 Plantarflexion – 4/5

🔬 Special Tests:

Anterior Drawer – Positive

Talar Tilt (Inversion in PF) – Positive

🚫 Ottawa Ankle Rules – Negative
